第一篇：妇产科晋升参考论文腹腔镜巨大子宫切除术78例体会作者：唐艳芬，王丹霞作者单位：北海市人民医院，广西北海，536000【摘要】目的：探讨使用双极电凝行腹腔镜巨大子宫切除术的安全性及手术技巧。方法：回顾分析2006年1月至2007年12月我院为78例子宫超过如孕12周患者行腹腔镜全子宫切除术的临床资料。术中置镜Trocar选在距宫底上至少3～4cm处。手术关键步骤是处理附件及游离子宫血管。结果：78例均在腹腔镜下完成手术，无一例中转开腹。2例术后出现阴道残端炎，余均无严重并发症发生。手术时间88～136min，平均112min，术中出血60～266ml，平均158ml，术后平均住院时间(5.1±0.9)d。随访6个月，恢复良好。结论：选择合适的置镜孔，处理好附件及子宫血管，使用双极电凝行腹腔镜巨大子宫切除术经济、安全、可行，不会增加手术危险性和并发症，但要求术者具备丰富的腹腔镜手术经验。【关键词】腹腔镜术;子宫切除术;双极电凝;并发症Laparoscopichysterectomyforlargeuterusbyusingbipolarcoagulation:areportof78casesTANGYanfen,WANGDanxia.Dept.ofGynecology,thePeople'sHospitalofBeihai,Beihai536000,China【Abstract】Objective:Toinvestigatetheoperativesafetyandskillsoflaparoscopichysterectomyoflargeuterusbyusingbipolarcoagulation.Methods:Theclinicaldataof78caseswhoseuteruswerebiggerthantwelvegestationalageuterusandunderwenttotallaparoscopichysterectomyfromJan.2006toDec.2007.Thelocationoflaparoscopewasatleast34cmabovethefundusofuterus.Themostcrucialstepwasthetreatmenttoadnexaanduterinebloodvessels.Results:Alllaparoscopicoperations(78cases)wereperformedsuccessfullyandnosevereoperativecomplicationswerenoticedexceptfor2casesofremnantvaginalendinflammation.Theaverageoperativetimeandtheintraoperativebloodlosswere112min(88136min)and158ml(60266ml)respectively.Theaveragepostoperativehospitalstaywas(5.1±0.9)days.Noseverepostoperativecomplicationwasfoundinallcasesduringthe6monthfollowup.Conclusions:Totallaparoscopichysterectomyforlargeuterusissafeandfeasiblebyusingbipolarcoagulation,anddoesnotincreaseoperativeriskandincidenceofcomplications,whensuitablelaparoscopiclocationisselectedandtreatmentsofadnexaanduterinevesselsarewellperformed.Theoperatorhavingrichlaparoscopicoperativeexperienceisthemostimportant.【Keywords】Laparoscopy;Hysterectomy;Bipolarcoagulation;Complication2006年1月至2007年12月我院为78例子宫如孕12～18周的患者行腹腔镜全子宫切除术，无并发症发生，效果满意。现报道如下。资料与方法1.1临床资料本组78例，42～55岁，平均(48.6±6.4)岁。子宫肌瘤67例，肌瘤数1～30枚，平均6枚，直径0.2～15cm，平均8cm;子宫腺肌症11例。子宫如孕12～14周57例，如孕14～16周18例，如孕16～18周3例。有腹部手术史8例，其中剖宫产术2例，附件切除术4例，阑尾
